Why Your Gut Health Matters More Than You Think in Perimenopause and Beyond — And What You Can Do About It

When we talk about menopause, the focus is almost always on hormones.

Oestrogen, progesterone, HRT, symptoms. These are all important, and they deserve proper attention. But there is a piece of the picture that is consistently overlooked, despite playing a central role in how women actually feel during this transition.
That piece is the gut.
In my clinical work, I rarely look at hormones in isolation — because what I see, time and again, is that the way a woman experiences perimenopause or menopause is deeply influenced by how well her gut is functioning. Some of the women I work with have already started HRT and are still struggling. Others are trying to manage symptoms through diet and lifestyle alone and feel like they're doing everything right but not getting the results they expected. In many of these cases, the gut is a significant part of the story that hasn't yet been explored.
The Gut–Hormone Connection in Perimenopause

Your gut is not simply a digestive organ. It is an active, intelligent system that is in constant communication with the rest of your body — including your hormonal systems.
One of the key ways this happens is through what researchers now call the estrobolome: the collection of gut bacteria specifically involved in metabolising and regulating oestrogen. Here is how it works in simple terms. Oestrogen that has been used by the body is sent to the liver, conjugated (essentially packaged for removal), and passed into the digestive tract to be excreted. Certain gut bacteria produce an enzyme called beta-glucuronidase, which can deconjugate oestrogen — effectively unpacking it and allowing it to be reabsorbed back into circulation.
When the estrobolome is balanced, this process is well regulated. The body clears what it needs to clear and recirculates what is appropriate. When gut bacteria are imbalanced — a state known as dysbiosis — this system can become dysregulated. Beta-glucuronidase activity may become excessive, leading to higher levels of oestrogen being reabsorbed. Or, in other cases, insufficient bacterial diversity means oestrogen is not processed efficiently at all. Either scenario can contribute to hormonal imbalance and make symptoms harder to manage.
This is not a fringe idea. Research into the estrobolome has grown significantly over the past decade, and it is increasingly recognised as an important factor in women's hormonal health — yet it is rarely discussed in the context of perimenopause care.

What Poor Gut Health Looks Like in Perimenopause
In my experience, I can often make a reasonable prediction about how a woman is likely to experience perimenopause based on her gut history and current digestive function. That is not because I am dismissing hormones — it is because the two are so closely linked.
When gut health is not well supported, I often see patterns such as:
Persistent bloating or digestive discomfort that worsens around hormonal fluctuations
Symptoms that do not fully resolve, even with HRT or other hormonal support
Heightened reactivity — including histamine sensitivity, skin flare-ups or increased inflammation
Mood changes, anxiety or low mood that seem disproportionate to what is happening hormonally
Nervous system dysregulation, driven in part by the gut-brain axis
A general sense that the body feels unpredictable or hard to read
None of this means hormones are the wrong focus. It means they are only part of the picture.
Why the Gut Becomes More Important During Perimenopause
Perimenopause is a time of significant physiological change and the gut is not immune to those changes.
Oestrogen itself has a direct influence on the gut microbiome. It affects intestinal permeability, gut motility and the diversity of bacterial populations. As oestrogen levels begin to fluctuate and eventually decline, the microbiome can shift in response. Research suggests that postmenopausal women tend to show reduced microbial diversity compared to premenopausal women — a change that has implications not just for digestion, but for immunity, inflammation, mood and metabolic health.
At the same time, several other factors that converge during midlife can compound this:
Stress tends to be higher during perimenopause — whether from life circumstances, disrupted sleep or the physical demands of hormonal change itself. Chronic stress directly impairs digestive function, reduces the diversity of beneficial bacteria and increases intestinal permeability.
Inflammation begins to play a larger role in midlife as oestrogen's natural anti-inflammatory effects diminish. A compromised gut can contribute to a low-grade inflammatory state that makes symptoms feel more intense and recovery slower.
Metabolic changes — including shifts in blood sugar regulation, insulin sensitivity and fat distribution — are closely tied to gut function. The microbiome plays a significant role in how we metabolise food, regulate appetite and manage energy.
What this means in practice is that a gut imbalance that may have been manageable in your thirties can become considerably more noticeable in your forties and fifties. The threshold shifts. The body has less reserve to compensate.
A Different Way of Thinking About Symptoms
It can be genuinely useful to reframe what symptoms are telling us. Rather than seeing digestive discomfort, mood changes, fatigue or hormonal sensitivity purely as a hormone problem to be corrected, we can start to ask a different question: what is the body struggling to process, regulate or respond to right now?
Often, the gut is part of the answer.
This is why some women feel significantly better when gut health is addressed — even if their hormone protocol remains exactly the same. The hormonal support was always appropriate. But the systems around it needed strengthening first.
How to Support Your Gut Health in Perimenopause
This does not need to be overwhelming. Consistent, meaningful shifts in the right direction tend to matter more than any single intervention.
Increase dietary diversity
A diverse microbiome requires diverse inputs. Aim to include a wide range of plant foods across the week — different fibres feed different bacterial populations, and variety is one of the most consistently supported strategies for gut health. In practice, this might mean rotating your vegetables rather than defaulting to the same few, adding herbs, spices, nuts and seeds more consistently, and paying attention to colour and texture as rough markers of variety.
Support digestion, not just what you eat
How you eat matters as much as what you eat — particularly if your digestive function is already under strain. Eating in a rushed or stressed state activates the sympathetic nervous system, which actively suppresses digestive capacity. Slowing down, chewing properly and creating some separation between stress and mealtimes are not small things. They can make a tangible difference to how well food is broken down and nutrients are absorbed.
Be thoughtful about ultra-processed foods
Highly processed foods — particularly those high in emulsifiers, additives and refined ingredients — have been shown to alter the composition of the gut microbiome over time. This does not mean that occasional processed food is harmful, but the overall balance of the diet matters. A foundation of mostly whole, minimally processed foods gives your microbiome a considerably better environment to thrive in.
Consider prebiotic-rich foods
Prebiotics are the fibres and compounds that selectively feed beneficial gut bacteria. Foods such as onions, garlic, leeks, oats, asparagus, chicory and flaxseeds are good sources. If your gut is currently sensitive or reactive, introducing these gradually tends to be better tolerated than adding large amounts at once.

Take your stress seriously as a gut issue
The gut and nervous system are in constant, bidirectional communication. Chronic stress reduces digestive enzyme production, alters the composition of gut bacteria, increases intestinal permeability and can drive a low-grade inflammatory response. Supporting your nervous system — through sleep, rest, movement, or whatever works for you individually — is not separate from gut health. It is a direct intervention on it.
Pay attention to your own patterns
There is no single gut protocol that works for every woman. Your body is offering you information constantly. Noticing which foods leave you feeling settled versus reactive, how your digestion changes across your cycle or in response to stress, and what your energy levels tell you after eating — all of this is useful clinical information, even before any testing has been done.

The steps above can make a meaningful difference for many women. But sometimes — particularly when symptoms are persistent, complex or not responding to general changes — a clearer picture is needed.

Where This Fits Alongside Hormone Support
To be clear: this is not about choosing between supporting your hormones and supporting your gut. The two work together.
Hormonal support — whether through HRT, herbal medicine or other means — can provide real relief and stability. But for that support to feel consistent and sustainable, the systems around it need to be functioning well. A well-supported gut improves how hormones are processed, cleared and recirculated. It reduces the inflammatory load that makes symptoms harder to bear. It supports the mood, energy and nervous system resilience that makes the whole transition more manageable.
When gut health is addressed alongside hormonal support, women often find that everything starts to feel more settled — not because the hormones have changed, but because the body is finally able to respond to them properly.

But for the body to respond well to hormonal support — whether through HRT or other means — the systems around it need to be functioning adequately. If the gut is inflamed, the microbiome is imbalanced, or digestive function is compromised, the body may struggle to process and utilise hormones effectively. In my clinical experience, addressing gut health alongside hormonal support often makes a meaningful difference to how well women feel overall.
